Address Update/Change Form

This form is to be used by defendants who wish to update their mailing address on file with the Court. The Court will use the information submitted below to send correspondence related to open/pending criminal and/or traffic matters not yet adjudicated before the Court.

The Court cannot accept future address changes or address changes that are not yet effective. As such, please wait until you are ready to receive mail at this new address to submit this form.

Please note that this form cannot be used to update your address to one outside of the fifty United States or the District of Columbia. Please call the Court for instruction on proceeding with an international address update.

Case/Docket Number *

If your Case Number ends in .01, .02, .03, etc., please only enter the 8 numbers preceding the decimal point. For example, if your number is 11112222.01, enter only 11112222 If you do not have this information readily available, please call the Court (914-670-3010) to obtain it.

Please use this form every time you move or have a change in your mailing address. The Court's case management system does not allow programmable, time-dependent address changes. As such, you will need to submit this form multiple times if you change your address several times during the pendency of your matter.
